      目的:研究振幅整合脑电图aEEG对新生儿缺氧缺血性脑病(Hypoxic ischemic encephalopathy,HIE)患儿病变程度及远期预 后的监测分析。方法:从2012 年3 月到2013 年3 月,48 例胎龄在37至41周的HIE 患儿,作为观察组。另选同期50 例住院治疗 的非颅脑病症足月儿作对照组。分别对两组行aEEG 诊断,对比两组连续性及SWC。并分析aEEG的监测结果和HIE 相关性,以 及远期预后情况。结果:观察组脑电图呈现为连续型者占比33.33%(16/48),SWC为成熟者占比20.83%(10/48),均显著低于对照 组的100%(50/50)及100%(50/50)。差异均有统计学意义(均P<0.05)。观察组的最高电压为56.54± 19.33 LV,显著高于对照组 的37.77± 2.79 LV;最低电压为4.26± 1.25 LV,显著低于对照组的7.75± 0.67 LV。差异均有统计学意义(均P<0.05)。依据 Spearman法行相关性分析后发现,aEEG的监测结果和HIE 临床分级呈正相关。随访6 个月,48 例患儿中,有11 例伤残(含智力 缺陷及脑瘫),占比22.92%。结论:aEEG 操作简便,诊断有效,且可连续监测,能够反应出新生儿HIE 病变程度以及远期预后。效 果显著,值得临床推荐。

      Objective:To study monitoring analysis of aEEG for Children patients with Hypoxic ischemic encephalopathy's grade of lesions and long-term prognosis.Methods:From March 2012 to March 2013, 48 cases of gestational age at 37-41 weeks' children with HIE were selected as the observation group. At the same period, another 50 cases of non- traumatic brain illness hospitalization term children were chosen as the control group. Two rows received respectively aEEG diagnosis, continuity and SWC. And ana- lyzed the aEEG monitoring results's relevance with HIE, and long-term prognosis condition.Results:The observation group's EEG showed a continuous type accounted for 33.33%(16/48), SWC mature persons accounted for 20.83%(10/48), which were significantly lower than the control group of 100%(50/50) and 100%(50/50). Differences were statistically significant (all P<0.05). The maximumvoltage of the observation group was 56.54± 19.33 LV, which was significantly higher than the control group of 37.77± 2.79 LV; mini- mum voltage was 4.26± 1.25 LV, which was significantly lower than the control group of 7.75± 0.67 LV. Differences were statistically significant (all P<0.05). According to Spearman correlation analysis found aEEG monitoring results with clinical grading of HIE had positive correlation. Followed for 6 months, 48 cases of children, in which had 11 cases of disability(included mental retardation and cerebral palsy), accounted for 22.92%.Conclusion:aEEG is a simple, effective diagnosis and continuous monitoring way, can reflect the severity of HIE and long-termprognosis. The effect is significant and is worthy of recommendation.
