      目的:通过锥形束CT 对脊柱侧弯患者下颌骨三维重建模型进行测量,分析下颌骨对称性,为进一步揭示脊柱侧弯与错牙合 畸形的相互关系提供参考依据。方法:对73 例脊柱侧弯患者(其中特发性38 例,先天性35 例)及40 例身体姿势无异常志愿者拍 摄锥形束CT 并经三维重建后,对下颌骨左右侧结构进行测量分析,对比不对称指数。结果:(1)先天性脊柱侧弯患者脊柱畸形及身 体姿势失平衡程度较特发性脊柱侧弯患者严重。(2)脊柱侧弯患者下颌骨体长度、下颌升支长度、下颌骨总长度不对称指数均较对 照组无明显差异(P>0.05)。(3)脊柱侧弯组中下颌角点成角不对称指数均较对照组高(P<0.05),先天性脊柱侧弯组髁突顶点成角 也明显高于对照组(P<0.05)。结论:脊柱侧弯患者下颌骨结构偏斜,提示脊柱形态及身体姿势对下颌骨发育可能存在一定影响。

      Objective:To evaluate the asymmetry of mandible in patients with scoliosis by using CBCT scan and 3-D reconstruction and find the relationship between scoliosis and mandibular malformation.Methods:A total of 73 scoliosis patients (38 cases of idiopathic scoliosis and 35 cases of congenital scoliosis) and 40 volunteers with normal body posture were taken cone-beam CT scan. After reconstruction of the mandible, the asymmetry index was assessed by measuring the difference between both sides and then was compared between the scoliosis group (idiopathic scoliosis group and congenital scoliosis group) and control group.Results:(1) Spinal deformity and trunk imbalance were more serious in congenital scoliosis group than in idiopathic scoliosis group. (2)No significant difference was found in asymmetry index of mandibular body length, ramus length and whole mandibular length(P>0.05). (3)There were significant differences between scoliosis group and control group in Co-Go-Pog Angle. And the asymmetry index value of Co-Co’-Go’ Angle was significant higher in congenital scoliosis group than that of the control group (P<0.05).Conclusion:Mandibular deviation was found in patients with scoliosis, suggesting that certain correlation might exist between mandibular development and scoliosis.
