| 目的：探讨2 种不同血液净化方式对慢性肾脏病（CKD）患者低血钙、高血磷及高血清甲状旁腺激素的改善效果。方法：选择
2011 年9 月到2014 年9 月在我院接收维持性血液透析（MHD）的患者64 例，随机分为HDF 组和HP-HD 组各32 例；HDF 组采
用血液透析滤过（HDF）治疗，HP-HD组采用血液灌流（HP）联合血液透析（HD）治疗；分别在治疗前及治疗后3 周采血检测血钙、
血磷和全段甲状旁腺激素（iPTH）。结果：治疗三周后，与治疗前相比较，两组患者血磷、iPTH显著下降，血钙显著上升，均有显著
性差异（P<0.05）；HP-HD 组患者血磷、iPTH下降较HDF组更为显著，均有显著性差异（P<0.05）。结论：HDF和HP 联合HD均能
有效调节血钙、清除血磷和iPTH 水平，但HP联合HD较HDF 效果更佳。 |

| Objective:To observe the effect of two kinds of different blood purification ways on hypocalcemia, hyperphosphatemia
and high serum parathyroid hormone of patients with chronic kidney disease(CKD) undergoing maintance hemodialysis.Methods:A total of 64 CKD patients, who underwent maintance hemodialysis in Ya'an People's Hospital of Sichuan Province from September
2011 to September 2014, were randomly divided into HDF group（n=32）and HP-HD group （n=32）. The HDF group were treated with
hemodiafiltration (HDF), while the HP-HD group, hemoperfusion plus hemodialysis(HP-HD). Before treatment and 3 weeks after treatment,
the blood calcium, blood phosphorus, and intact parathyroid hormone (iPTH) were detected respectively.Results:After 3 weeks of
treatment, blood calcium in the two groups increased, while blood phosphorus and iPTH decreased, compared with before treatment, the
differences were statistically significant（P<0.05）. In addition, blood phosphorus and iPTH clearance rate of HP-HD group was obviously
greater than those of the HDF group with statistically significant difference（P<0.05）.Conclusion:Both HDF and HP combined with HD
can effectively regulate the levels of blood calcium or remove blood phosphorus and iPTH, but the HP combined with HD has better effect. |
