      目的:探讨膝下下肢动脉硬化闭塞症(ASO)患者行球囊扩张成形术前后血浆中内皮素-1(ET-1)和一氧化氮(NO)的水平变 化及其临床意义。方法:收集我院血管外科2013 年2 月至2014 年2月收治的行球囊扩张成形术的膝下ASO 患者38 例,比较术 前和术后6 h、24 h、1周、1个月和3个月的血浆NO和ET-1 水平变化,患者术后1 个月和3 个月行CT 血管造影术(CTA)复查, 判断是否发生血管再狭窄,并分析血管再狭窄与血浆NO和ET-1 的关系。结果:所有患者术后6h 血浆ET-1 较术前显著升高 (P<0.05),术后24 h至1周略有下降但仍维持在较高水平,NO变化与ET-1 相反;CTA 检查未发现血管再狭窄者33 例,血浆 ET-1 和NO在术后1 个月恢复至术前水平,并维持至术后3 个月;而CTA 检查发现血管再狭窄者5 例,血浆ET-1 术后1 个月至 3 个月仍维持在较高水平,血浆NO水平变化与ET-1 相反。结论:血浆NO 和ET-1 与膝下ASO 患者球囊扩张成形术后血管再 狭窄相关。

      Objective:To investigate the changes of plasma endothelin-1 (ET-1) and NO before and after balloon angioplasty in patients with atherosclerotic occlusive disease (ASO) below the knee and its clinical significance.Methods:A total of 38 patients with ASO below the knee, who underwent balloon angioplasty in department of vascular surgery of Tsitsihar First Hospital from February 2013 to February 2014,were selected. The changes of plasma ET-1 and NO levels before operation and 6 hours, 24 hours, 1 week, 1 month and 3 months after operation were compared. All the patients were checked with CT angiography(CTA) 1 month and 3 months after operation respectively to observed whether vascular restenosis occurred or not,and the relationship between vascular restenosis and plasma ET-1 and NO levels was analyzed.Results:Compared with before operation,the plasma ET-1 levels 6 hours after operation increased significantly and decreased slightly from 24 hours to 1 week after operation in all the patients ,but still kept at a high level, while the changes of NO was opposite to plasma ET-1.There were 33 patients without vascular restenosis after checked with CTA, the plasma ET-1 and NO levels resumed to preoperative level 1 month after operation and kept till 3 months after operation; however, there were 5 patients with vascular restenosis,the plasma ET-1 level kept at high level from 1 month to 3 months after operation, and the changes of NO was opposite to plasma ET-1 level.Conclusion:The plasma ET-1 and NO levels was related with vascular restenosis after balloon angioplasty in patients with ASO below the knee.
