      目的:探讨谷氨酰胺对胃癌术后血清VEGF、IL-8及肿瘤标志物水平影响。方法:收集我院胃癌需进行胃癌根治术患者92 例,随机分为实验组和对照组。两组患者均根据自身病情进行胃癌根治术。术后第1 天予250 mL 生理盐水+2 mL亚甲蓝确认无 反流后,对照组予以5 %葡萄糖溶液500 mL泵入,第2 天予能全力500 mL,第3 天开始予以能全力1500 mL泵入;实验组在对照 组基础上加用谷氨酰胺。两组均术前及术后第10天进行血清VEGF、IL-8 及肿瘤标志物CEA、CA72-4、CA19-9水平检测。并于术 前及术后1 个月进行疼痛评估和生存质量评分。结果:①术后第10 天两组患者血清VEGF、IL-8 及肿瘤标志物CEA、CA72-4、 CA19-9 水平均下降,与对照组比较,实验组下降均更明显,差异有统计学意义(P<0.05);② 术后1 个月两组患者VRS 疼痛等级 均下降,生存质量WHOQOL 评分均上升,与对照组比较,实验组无疼痛人数明显增多、生存质量评分更高,差异有统计学意义 (P<0.05)。结论:谷氨酰胺可更显著改善胃癌术后患者血清VEGF、IL-8及肿瘤标志物水平,对提高患者机体免疫力、改善营养状 态、提高患者生活质量有重要意义。

      Objective:To investigate the effect of glutamine on the serum levels of VEGF, IL-8 and tumor markers in postoperative gastric cancer patients.Methods:92 patients undergoing radical gastroectomy for gastric cancer from our hospital were selected and randomly divided into the control group and the experiment group. The patients were performed according to their own conditions for gastric cancer radical operation. After first day of operation, the 250 mL normal saline +2 mL methylene blue was confirmed to have no reflux, and then the patients in control group were treated with 500 mL 5 %glucose solution, and pumped into1500 mL nutrison liquid on 2nd day and 1500 mL nutrison liquid on 3rd day. On the basis of the control group, the experimental group was treated with glutamine. The serum levels of VEGF, CEA and tumor markers IL-8, CA72-4 and CA19-9 were tested and compared on the tenth day after surgery. The pain assessment and quality of life score were compared in preoperative and postoperative 1 month.Results:Compared with before treatment, the serum levels of VEGF, CEA and tumor markers IL-8, CA72-4 and CA19-9 were lower in the tenth day after surgery(P<0.05), and the decrease of these indexes in the experiment group were more obviously(P<0.05). Compared with before treatment, the VRS pain rating were lower in 1 month after operation(P<0.05), and the WHOQOL score were higher(P<0.05), and the experiment group were superior to the control group (P<0.05).Conclusion:Glutamine can significantly improve serum VEGF, IL-8 and tumor markers in patients with gastric cancer after operation, which is important to improve the body immunity, improve the nutritional status and the life quality of patients.
