      目的:探讨微创经皮钢板接骨术(Minimally invasive percutaneous plate osteosynthesis, MIPPO)技术治疗高能量胫骨干骨折的 临床疗效。方法:选择2013年7 月-2015 年7 月在我院接受手术治疗的高能量胫骨干骨折患者57 例作为研究对象,根据手术方 法不同将其分为MIPPO组(34 例)和对照组(23 例)。MIPPO组患者应用微创经皮钢板接骨术治疗,对照组患者采用传统内固定 术治疗。观察并比较两组患者的手术效果及并发症的发生情况。结果:MIPPO 组患者手术优良率为85.29%(29/34),对照组患者手 术优良率为69.57%(16/23);MIPPO 组手术效果明显优于对照组,差异具有统计学意义(P<0.05)。与对照组比较,MIPPO 组患者 的手术时间短、术中出血量少、骨折愈合时间早,差异均具有统计学意义(P<0.05)。两组患者均发生内固定松动、切口感染、骨恢复 畸形等并发症,其中MIPPO 组并发症的发生率为5.88%(2/34),对照组并发症的发生率为8.71%(4/23),MIPPO 组并发症的发生 率明显低于对照组,差异具有统计学意义(P<0.05)。结论:MIPPO技术治疗高能量胫骨干骨折具有明显的临床效果,且并发症的 发生率较低,安全性较高,值得临床推广应用。

      Objective:To explore the clinical efficacy of minimally invasive percutaneous plate osteosynthesis on the treatment of high energy tibial fractures.Methods:57 cases with high energy tibial shaft fractures who were treated in our hospital from July 2013 to July 2015 were selected as the study subjects. According to the different surgical methods, the patients were divided into the MIPPO group (34 cases) and the control group (23 cases). The patients in the MIPPO group were treated with the minimally invasive percutaneous plate osteosynthesis surgery, while the patients in the control group were treated with the traditional internal fixation surgery. Thenj the clinical effects and the incidence of complications in the two groups were observed and compared.Results:The excellent rate of patients in the MIPPO group was 85.29%(29/34), which is higher than 69.57%(16/23) in the control group, and the difference was statistically significant (P<0.05). Compared with the control group, the operation time was shorter, the blood loss was less, the fracture healing time was earlier of the patients in the MIPPO group, and the difference was statistically significant (P<0.05). There were fixation loosening, infection, bone restoration deformity and other complications in both two groups. The incidence of postoperative complications in the MIPPO group was 5.88%(2/34), which was lower than 8.71%(4/23) in the control group, and the difference was statistically significant (P<0.05).Conclusion:MIPPO has obvious clinical effect on the treatment of high energy tibial shaft fractures with lower incidence of complications and higher safety, which is worthy of clinical application.
