| 目的：研究2 型糖尿病伴脂肪肝患者血浆成纤维细胞生长因子21（FGF21）水平与肥胖、脂代谢及胰岛素抵抗的相关性，为
临床诊疗提供依据。方法：选取2013 年5 月到2015 年11 月我院收治的2 型糖尿病伴脂肪肝患者100 例为研究组，另选取同期
单纯脂肪肝患者100 例为脂肪肝组，健康体检者100 例为对照组，比较各组入选次日清晨FGF21、谷丙转氨酶（ALT）、谷草转氨酶
（AST）、胆固醇（TC）、甘油三酯（TG）、低密度脂蛋白胆固醇（LDL-C）、高密度脂蛋白胆固醇（HDL-C）、游离脂肪酸（FFA）、体重指
数（BMI）、腰臀比（WHR）、空腹胰岛素（FINS）、空腹血糖（FBG）以及胰岛素抵抗指数（HOMA-IR）。结果：研究组TG、TC、AST、
ALT、LDL-C、FFA、FBG、BMI、WHR、FINS、HOMA-IR 以及FGF21 均显著高于对照组，HDL-C显著低于对照组，比较差异具有统
计学意义（P＜0.05）；研究组FFA、TG、FINS、FBG、HOMA-IR 以及FGF21显著高于脂肪肝组，BMI和WHR显著低于脂肪肝组，比
较差异具有统计学意义（P＜0.05）；相关性分析显示：FGF21 与TG、FFA、BMI以及HOMA-IR 呈正相关关系（P＜0.05）。结论：2 型
糖尿病合并脂肪肝患者FGF21 水平会显著升高，且与脂肪代谢、肥胖以及胰岛素抵抗有关。 |

| Objective:To study the correlation between plasma fibroblast growth factor 21 (FGF21) levels and obesity, lipid
metabolism and insulin resistance in type 2 diabetic patients with fatty liver disease, and to provide the basis for clinical diagnosis and
treatment.Methods:100 cases of type 2 diabetic patients with fatty liver disease were selected in our hospital from May 2013 to November
2015 as study group, another 100 cases of patients with fatty liver were selected as fatty liver group at the same period, anf 100 cases
of healthy person who accepted the physical examination as the control group, FGF21, alanine aminotransferase (ALT), aspartate aminotransferase(
AST), total cholesterol(TC), triglyceride (TG), low density lipoprotein cholesterol(LDL-C), high density lipoprotein cholesterol
(HDL-C), free fatty acid(FFA), body mass index(BMI), waist to hip ratio(WHR), fasting insulin(fins), fasting blood glucose(FBG)
and insulin resistance index (HOMA-IR) the next day morning in each groups were compared.Results:The TG, TC, AST, ALT, LDL-C,
FFA, FBG, BMI, WHR, FINS, HOMA-IR and FGF21 levels in the study group were significantly higher than the control group, while the
HDL-C was significantly lower than the control group, the differences were statistically significant (P<0.05); The FFA, TG, FINS, FBG,
HOMA-IR and FGF21 in the study group were significantly higher than the fatty liver group, the BMI and WHR were significantly lower
than those in the fatty liver group, the differences were statistically significant(P<0.05); Correlation analysis showed that FGF21 was positively
correlated with TG, FFA, BMI and HOMA-IR (P<0.05).Conclusion:FGF21 levels in type 2 diabetic patients with fatty liver are
significantly elevated, and is related to fat metabolism,obesity and insulin resistance. |
