      摘要 目的:探讨肿瘤坏死因子-α(TNF-α)和白细胞介素-1β(IL-1β)在干眼症(DES)患者结膜上皮细胞和泪液中的表达及意义。方法:选择57例DES患者和50例健康者分别作为DES组和对照组,比较两组结膜上皮细胞和泪液中TNF-α、IL-1β基线值的差异,比较DES组治疗后结膜上皮细胞和泪液中TNF-α、IL-1β值与基线值的差异,治疗前后临床检查情况(泪膜破裂时间、泪液分泌试验、角膜荧光素染色),并对TNF-α、IL-1β表达和临床检查进行相关性分析。结果:DES组泪液、结膜上皮细胞中TNF-α、IL-1β表达均明显高于对照组(P<0.05)。DES组治疗后泪液、结膜上皮细胞中TNF-α、IL-1β表达均较治疗前明显降低(P<0.05)。与治疗前相比,DES患者治疗后泪膜破裂时间、泪液分泌试验、角膜荧光素染色等临床检查明显改善(P<0.05)。TNF-α、IL-1β表达与泪膜破裂时间、泪液分泌试验呈负相关(P<0.05),与角膜荧光素染色呈正相关(P<0.05)。结论:DES患者泪液、结膜上皮细胞中TNF-α与IL-1β的表达上调,且与干眼症的病情进展密切相关,可为干眼症的临床诊断、治疗或预后提供参考。

      ABSTRACT Objective: To explore the expressions and significances of tumor necrosis factor -α(TNF-α) and interleukin-1β(IL-1β) in the conjunctival epithelial cells and tear of patients with dry eye syndrome(DES). Methods: 57 cases of DES patients and 50 cases of healthy persons were selected to as the DES group and the control group. The baseline values of TNF-α and IL-1β in the conjunctival epithelial cells and tear were compared between the DES group and the control group, and the values of TNF-α and IL-1β in the conjunctival epithelial cells and tear and the clinical examination(tear film rupture time, tear secretion test, corneal fluorescein staining) of DES patients before and after treatment were compared, the relationship between TNF-α and IL-1β levels in the conjunctival epithelial cells and tear and clinical examination (tear break-up time, tear secretion test, corneal fluorescein staining) were analyzed. Results: The expressions of TNF-α and IL-1β in the DES group were significantly higher than those of the control group (P < 0.05). After treatment, the expressions of TNF- α and IL-1β in the lacrimal and conjunctival epithelial cells of the DES group were significantly decreased(P<0.05). Compared with before treatment, the clinical examination(tear film rupture time, tear secretion test, corneal fluorescein staining) of DES patients significant improved(P<0.05). The expressions of TNF-α and IL-1β were negatively correlated with the tear break-up time and tear secretion test(P<0.05) and positively correlated with the corneal fluorescein staining(P<0.05). Conclusion: The expressions of TNF-α and IL-1β are closely related to the development of dry eye, and may be involved in the pathogenesis of dry eye.
