      摘要 目的:探讨骨化三醇对IgA肾病患者血清无机盐离子、肾小管间质损伤尿液生物学标志物及肾小球功能的影响。方法:选取我院收治的70例IgA肾病患者,根据数字随机分组的方式分为对照组(35例)和观察组(35例)。对照组给予厄贝沙坦片,观察组给予厄贝沙坦片和骨化三醇胶丸。比较两组治疗前后血钾(K)、血钙(Ca)、血磷(P)、α1微球蛋白(α1-MG)、N-乙酰-β-D氨基葡萄糖苷酶(NAG)、尿β2微球蛋白(β2-MG)及尿微量白蛋白(U-mAlb)水平的变化。结果:治疗后,两组血K、Ca、P含量与治疗前相比差异均无统计学意义(P>0.05),且组间比较差异亦不显著(P>0.05);观察组肾小管间质损伤尿液生物学标志物(血清α1-MG、NAG)水平均较治疗前显著降低(P<0.05),且均明显低于对照组(P<0.05);两组肾小球功能相关因子(血清β2-MG、U-mAlb)水平较治疗前均显著降低(P<0.05),且观察组以上指标均明显低于对照组(P<0.05)。结论:厄贝沙坦联合骨化三醇治疗IgA肾病能有效减少肾小管损伤,并改善肾小球功能。

      ABSTRACT Objective: To investigate the effects of calcitriol on the serum inorganic salt ions, urinary biomarkers of renal tubulointerstitial injury and glomerular function in patients with IgA nephropathy were investigated. Methods: 70 cases of patients with IgA nephropathy in our hospital were selected and divided into the control group (35 cases) and the observation group (35 cases) according to the number random grouping method. The control group was given Irbesartan Tablets, while the observation group was given Irbesartan Tablets and Calcitriol Capsules. The levels of blood potassium (K), blood calcium (Ca), blood phosphorus (P), serum alpha 1 microglobulin (α1-MG), N-acetyl-beta-D aminoglucosidase (NAG), urinary beta 2 microglobulin (β2-MG) and urine microalbumin (U-mAlb) levels of two groups were compared before and after treatment. Results: After treatment, the levels of blood K, Ca, and P in the two groups showed no significant difference compared with those before treatment(P>0.05), and no stastical difference was found between the two groups(P>0.05). After treatment, the urine biomarkers(α1-MG, NAG) of tubulointerstitial injury in the observation group were significantly lower than those before treatment(P<0.05), and they were significantly lower than those of the control group (P<0.05). After treatment, the levels of glomerular function-related factors(β2-MG, U-mAlb) in the two groups were significantly lower than those before treatment(P<0.05), and they were significantly lower in the observation group than those of the control group (P<0.05). Conclusion: Irbesartin combined with calcitriol can effectively reduce renal tubular injury and improve the glomerular function in the treatment of IgA nephropathy.
