      目的:探讨血管内皮生长因子受体2(VEGFR2)、可溶性内皮因子(sEng)联合子宫动脉血流参数[血流阻力指数(RI)、收缩期与舒张期的血流速度比值(S/D)、血流搏动指数(PI)]对子痫前期患者胎儿生长受限(FGR)的预测价值。方法:选取2020年1月到2024年9月期间我院收治的136例子痫前期患者。均接受超声检查,记录子宫动脉血流参数。根据FGR发生情况分为FGR组(n=42)和正常组(n=94),比较FGR组和正常组的子宫动脉血流参数、血清VEGFR2、sEng水平。分析子宫动脉血流参数与血清VEGFR2、sEng水平的相关性。受试者工作特征(ROC)曲线分析血清VEGFR2、sEng水平联合子宫动脉血流参数对子痫前期患者FGR的预测价值。结果:FGR组的RI、S/D、PI高于正常组(P<0.05)。FGR组的血清sEng高于正常组,血清VEGFR2低于正常组(P<0.05)。Pearson相关性分析结果显示,血清VEGFR2与RI、PI、S/D呈负相关,而血清sEng与RI、PI、S/D呈正相关(P<0.05)。RI、PI、S/D、VEGFR2、sEng单独及联合应用时曲线下面积(AUC)分别为0.792、0.761、0.822、0.788、0.887、0.971,其中联合应用预测效能最高。结论:RI、PI、S/D联合血清VEGFR2、sEng对子痫前期患者FGR的预测价值较高。

      Objective: To investigate the predictive value of serum vascular endothelial growth factor receptor 2 (VEGFR2) and soluble endothelial factor (sEng) combined with uterine artery blood flow parameters [blood flow resistance index (RI), systolic/diastolic blood flow velocity ratio (S/D) , blood flow pulsation index (PI) ] for fetal growth restriction (FGR) in preeclampsia patients. Methods: 136 preeclampsia patients who were admitted in our hospital from January 2020 to September 2024 were selected. All patients underwent ultrasound examination, and uterine artery blood flow parameters were recorded. Patients were divided into FGR group (n=42) and normal group (n=94) according to whether FGR occurred, the uterine artery blood flow parameters, serum VEGFR2 and sEng levels were compared between FGR group and normal group. The correlation between uterine artery blood flow parameters and serum VEGFR2, sEng levels were analyzed. The predictive value of VEGFR2 and sEng levels combined with uterine artery blood flow parameters for FGR in preeclampsia patients were analyzed by receiver operating characteristic (ROC) curve. Results: The RI, S/D, PI in FGR group were higher than those in normal group (P<0.05). Serum sEng in FGR group was higher than that in normal group, and serum VEGFR2 was lower than that in normal group (P<0.05). Pearson correlation analysis showed that, serum VEGFR2 was negatively correlated with RI, PI and S/D, and serum sEng was positively correlated with RI, PI and S/D (P<0.05). The area under the curve (AUC) of RI, PI, S/D, VEGFR2, sEng alone and in combination were 0.792, 0.761, 0.822, 0.788, 0.887 and 0.971, respectively, the combined application had the highest predictive efficiency. Conclusion: RI, PI, S/Dcombined with serum VEGFR2 and sEng have higher predictive value for FGR in preeclampsia patients.
