| 目的：探讨醒脑开窍针刺法对肝气郁结型产后抑郁患者抑郁症状、神经内分泌和自噬标志物表达的影响。方法：采用随机数字表法将我院2022年3月~2024年2月期间收治的肝气郁结型产后抑郁患者（151例）分为实验组（n=76，西医治疗联合醒脑开窍针刺法）、对照组（n=75，西医治疗）。对比两组中医疗效以及治疗前后的抑郁相关量表评分、神经内分泌指标、自噬标志物。结果：实验组的临床总有效率高于对照组（P<0.05）。两组治疗后汉密尔顿抑郁量表-17项版本（HAMD-17）评分、爱丁堡产后抑郁量表（EPDS）评分、血清自噬降解底物蛋白1（p62）水平、血清皮质醇、促肾上腺皮质激素（ACTH）水平下降，且实验组低于对照组（P<0.05）。两组治疗后血清微管相关蛋白轻链3-Ⅱ（LC3-Ⅱ）、自噬基因BECN 1（Beclin1）水平、血清5-羟色胺（5-HT）水平升高，且实验组高于对照组（P<0.05）。结论：醒脑开窍针刺法用于肝气郁结型产后抑郁患者，可有效缓解抑郁症状，调节神经内分泌指标和自噬标志物。 |

| Objective:To explore effects of Xingnao?Kaiqiao?acupuncture?treatment on depressive symptoms, neuroendocrine and autophagy markers expression in postpartum depression patients with stagnation of liver qi.Method: Using random number table method, postpartum depression patients with stagnation of liver qi (151cases) who were admitted to our hospital from March 2022 to February 2024 were divided into experimental group (n=76, western medicine treatment combined with Xingnao?Kaiqiao?acupuncture?treatment) and control group (n=75, western medicine treatment) .The therapeutic effects of traditional Chinese medicine, as well as the depression related scale scores, neuroendocrine indicators, and autophagy markers of two groups before and after treatment were compared. Result: The clinical total effective rate of the experimental group was higher than that of the control group (P<0.05). The Hamilton Depression Rating Scale 17 (HAMD-17) score, Edinburgh Postnatal Depression Scale (EPDS) score, serum autophagic degradation substrate protein 1 (p62) level, serum cortisol, and adrenocorticotropic hormone (ACTH) levels decreased after treatment in both groups, and the experimental group was lower than that of the control group (P<0.05). The levels of serum microtubule associated protein light chain 3-II (LC3-II), autophagy gene BECN 1 (Beclin1), and serum serotonin (5-HT) increased after treatment in both groups, and the experimental group was higher than that of the control group(P<0.05).Conclusion: Xingnao?Kaiqiao?acupuncture?treatment can effectively alleviate depressive symptoms, regulate neuroendocrine indicators and autophagy markers in postpartum depression patients with stagnation of liver qi. |
