目的 观察曲克芦丁脑蛋白水解物联合高压氧、青霉素在神经梅毒认知障碍患者中的应用效果。方法 纳入我院2019年4月至2023年12月期间收治的神经梅毒认知障碍患者60例,采随机数字表法将患者分为A组、B组和C组三组,每组各20例;其中A组采用青霉素治疗,B组采用青霉素联合高压氧治疗,C组采用青霉素、高压氧联合曲克芦丁脑蛋白水解物注射液治疗。对比三组在治疗前、后的简易精神状态评价(MMSE)评分、血清甲苯胺红不加热血清试验(TRUST)滴度检查及不良反应发生率。结果 三组治疗后MMSE评分升高,且A组、B组、C组依次升高(P<0.05)。除了1例1:2滴度、2例1:32滴度对象在治疗前后TRUST滴度无变化外,其余患者血清TRUST均下降,部分患者下降明显,治疗后在1:16滴度区以下聚集大部分患者,未出现驱梅后TRUST滴度不降反升者,尤其对于滴度较高者下降明显。三组不良反应发生率组间对比未见差异(P>0.05)。结论 单纯青霉素驱梅治疗有利于改善神经梅毒患者的认知障碍情况,联合高压氧及曲克芦丁脑蛋白水解物注射液治疗后患者的认知水平改善更明显。 |

Objective: To observe the application effect of troxerutin brain protein hydrolysate injection combined with hyperbaric oxygen and penicillin in patients with neurosyphilis cognitive impairment. Methods: 60 patients with neurosyphilis cognitive impairment who were admitted to our hospital from April 2019 to December 2023 were included, patients were divided into A group, B group and C group by random number table method, with 20 cases in each group. A group was treated with penicillin, B group was treated with penicillin combined with hyperbaric oxygen, and C group was treated with penicillin, hyperbaric oxygen combined with troxerutin brain protein hydrolysate injection. The simple mental state evaluation (MMSE) score, serum toluidine red unheated serum test (TRUST) titer examination and the incidence of adverse reactions were compared among three groups before and after treatment. Results: The MMSE scores in three groups increased after treatment, and the scores in A group,B group and C group increased in turn (P<0.05). Except that 1 case of 1:2 titer and 2 cases of 1:32 titer showed no change in TRUST titers before and after treatment, the rest of the patients serum TRUST were decreased, some patients decreased significantly, most of the patients were gathered below the 1:16 titer area after treatment, there was no increase in TRUST titer after expelling plum, especially for those with higher titer decreased significantly. There was no difference in the incidence of adverse reactions in three groups (P>0.05). Conclusion: Penicillin alone for plum treatment is beneficial to improve the cognitive impairment of patients with neurosyphilis, and the improvement of cognitive level of patients after combined with hyperbaric oxygen and troxerutin brain protein hydrolysate injection solution is more obvious. |
