摘要 目的:探讨妊娠早期血清同型半胱氨酸(Hcy)、血清铁蛋白(SF)水平对妊娠期糖尿病(GDM)的预测价值。方法:按妊娠24~28周75 g口服葡萄糖耐量试验(75 g OGTT)结果,将412例孕妇分为GDM组(n=96)与糖耐量正常(NGT)组(n=316)。收集孕妇一般资料,于妊娠6~14周行血清Hcy、铁蛋白水平检测,比较两组临床资料差异,使用Logistic筛选GDM的危险因素,并使用受试者工作特征(ROC)评价相关指标对GDM的预测能力。结果:较于NGT组,GDM组孕妇年龄较大,妊娠前BMI、空腹血糖、糖化血红蛋白、三酰甘油水平和血清Hcy、铁蛋白水平更高(P<0.05)。Logistic回归分析得出,年龄较大、妊娠前BMI较高、HbA1c水平较高及血清Hcy水平较高、铁蛋白水平较高均是GDM的危险因素(P<0.05)。ROC曲线分析得出,血清Hcy、铁蛋白对GDM均有一定预测能力,其AUC分别为0.753、0.746。联合预测模型的预测效能更大,其AUC高达0.917。结论:妊娠早期血清Hcy、铁蛋白水平增高与GDM发病相关,可作为GDM的预测生物标志物。 |

ABSTRACT Objective: To investigate the predictive value of serum homocysteine (Hcy) and ferritin(SF) levels in early pregnancy for gestational diabetes mellitus (GDM). Methods: According to the results of 75 g oral glucose tolerance test (75 g OGTT) at 24 ~ 28 weeks of pregnancy, 412 pregnant women were divided into GDM group (n=96) and normal glucose tolerance (NGT) group (n=316). The general data of pregnant women were collected, and the levels of serum Hcy and ferritin were detected at 6-14 weeks of pregnancy. The two groups's clinical data were compared. The risk factors of GDM was analysed by Logistic analysis, and the receiver operating characteristic (ROC) was used to evaluate the predictive ability of relevant indicators for GDM. Results: Compared with the NGT group, the pregnant women in the GDM group were older, and the levels of BMI, fasting blood glucose, glycosylated hemoglobin, triglyceride, serum Hcy and ferritin before pregnancy were higher(P<0.05). Logistic regression analysis showed that older age, higher pre-pregnancy BMI, higher HbA1 c level, higher serum Hcy level and higher ferritin level were risk factors for GDM(P<0.05). ROC curve analysis showed that serum Hcy and ferritin had certain predictive ability for GDM, and their AUCs were 0.753 and 0.746, respectively. The prediction efficiency of the combined prediction model is greater, and its AUC is as high as 0.917. Conclusion: The increase of serum Hcy and ferritin levels in early pregnancy is related to the pathogenesis of GDM, which can be used as a predictive biomarker for GDM. |
