      目的:探讨上海地区2008 与2012 年泌尿生殖道解脲脲原体(Ureaplasma urealyticum,UU)耐药性变化,为临床合理用药提供 参考。方法:采用自制UU药物敏感检测试剂对2008 年450 例和2012 年459 例患者的标本进行检测,观察UU阳性情况及UU 对交沙霉素、氧氟沙星、阿奇霉素和强力霉素的药物敏感性。结果:2008 年分离到150 例UU阳性标本和2012 年分离到134 例 UU 阳性标本分别对交沙霉素、强力霉素、氧氟沙星和阿奇霉素等4 种抗菌素的敏感率有显著性差异;2008 年46 例和2012 年38 例男性患者阳性标本中分离的UU分别对交沙霉素和阿奇霉素敏感率有显著性差异; 对强力霉素和氧氟沙星敏感率无显著性差 异。2008 年104 例和2012 年96 例女性患者阳性标本中分离的UU分别对交沙霉素、强力霉素、氧氟沙星和阿奇霉素等4 中抗菌 素的敏感率都有显著性差异。结论:5 年间UU药物敏感性发生了变迁;临床医师应该关注本地区药物敏感性变迁,合理选用抗生 素。

      Objective:To identify the trend of antibiotic sensitivity and resistance patterns among Ureaplasma urealyticum urethral infections in both males and females from 2008 to 2012.Methods:Patients were identified on the basis of clinical presentation (dysuria, polyuria, urethral discharge) and urethral cultures were obtained. From those patients that tested positive for Ureaplasma urealyticum, specimens were isolated. With the addition of laboratory self-generated Mycoplasma reagent, susceptibilities were determined to the following four antibiotics: josamycin, azithromycin, olfloxacin, and doxycycline.Results:In 2008, 450 patients were identified with symptoms suggestive of urethritis. Among them, 150 (46 male and 104 female) cultured Ureaplasma urealyticum. In 2012, 459 patients were identified with symptoms suggestive of urethritis. 134 (38 male and 96 female) of these patients cultured Ureaplasma urealyticum. Among the male patients, a statistically significant difference (P<0.05) in the rate of sensitivity to azithromycin and josamycin was observed in 2008 when compared with that of 2012. Doxycycline and olfoxacin displayed no such statistical difference in sensitivity rate among males in 2008 compared to 2012. Among the female patients, a statistically significant difference (P<0.05) in the rate of sensitivity to all four antibiotics (azithromycin, doxycycline, olfloxacin, and josamycin) was observed in 2008 when compared with 2012.Conclusion:The trend in antibiotic sensitivity of Ureaplasma urealyticum to azithromycin (males and females), doxycycline (females), olfloxacin (females), and josamycin (males and females) has changed to a statistically significant degree from2008 to 2012. Doctor must emphasize on the trends pathogen flora and drug-resistance of local area patients, in order to direct drug use in clinical practice.
