      摘要 目的:探讨轻中度阿尔茨海默病(AD)患者采用盐酸多奈哌齐片、甘露特钠胶囊联合治疗后,患者神经递质、认知功能和肠道菌群的变化情况。方法:采用随机数字表法将我院2020年3月~2023年3月期间收治的102例AD患者分为对照组(n=51,盐酸多奈哌齐片治疗)和研究组(n=51,甘露特钠胶囊联合盐酸多奈哌齐片治疗)。对比两组患者相关量表评分、神经递质和肠道菌群的变化。结果:研究组治疗6个月后简易智能精神量表评分(MMSE)评分、日常生活能力量表(ADL)评分、血清乙酰胆碱(Ach)、5-羟色胺(5-HT)、谷氨酸(Glu)、粪便双歧杆菌、乳酸杆菌数量高于对照组,痴呆病理行为评定表(BEHAVE-AD)、粪便梭状芽胞杆菌、葡萄球菌、假单胞菌、变形杆菌数量低于对照组(P<0.05)。结论:轻中度AD患者采用盐酸多奈哌齐片、甘露特钠胶囊联合治疗,可改善认知功能,调节神经递质和肠道菌群。

      ABSTRACT Objective: To investigate the changes in neurotransmitters, cognitive function, and gut microbiota in patients with mild to moderate Alzheimer's disease (AD) treated with a combination of donepezil hydrochloride tablets and mannitol sodium capsules. Methods: The random number table method was used, 102 AD patients admitted to our hospital from March 2020 to March 2023 were divided into control group (n=51, treated with donepezil hydrochloride tablets) and study group (n=51, treated with ganlute sodium capsules combine with donepezil hydrochloride tablets). The related scale scores, neurotransmitters and intestinal flora in two groups were compared. Results: The score of Mini-Mental State Examination (MMSE), Activity of Daily Living (ADL), serum acetylcholine (Ach), 5-hydroxytryptamine (5-HT) and glutamic acid (Glu), number of fecal Bifidobacterium and Lactobacillus in study group was higher than that in control group 6 months after treatment, and the scores of Behavioral Pathology of Dementia (BEHAVE-AD), number of fecal Clostridium, Staphylococcus, Pseudomonas and Bacillus proteus in study group were lower than those in control group (P<0.05). Conclusion: Combination treatment of donepezil hydrochloride tablets and mannitol sodium capsules for mild to moderate AD patients, can improve cognitive function, regulate neurotransmitters and gut microbiota.
