      目的:探讨肋骨骨折患者在右美托咪定联合罗哌卡因竖脊肌平面阻滞(ESPB)下,患者术后免疫功能、镇痛效果和应激反应的变化。方法:以随机数字表法将我院于2022年6月~2023年6月期间收治的104例肋骨骨折患者分为观察组、对照组,各52例。两组患者均进行手术治疗,对照组采用罗哌卡因ESPB镇痛,观察组采取右美托咪定联合罗哌卡因ESPB镇痛。对比两组术后镇痛效果、认知功能评分、血清应激反应指标、免疫功能和不良反应。结果:与对照组术后1h、3h、6h、12h、24h 相比,观察组活动状态及静息状态的视觉模拟量表(VAS)评分更低,简易智力状态检查量表(MMSE)评分更高(P<0.05)。与对照组术后24h相比,观察组血清皮质醇(Cor)、甲肾上腺素(NE)、肾上腺素(E)、CD4+、CD3+、CD4+/CD8+更高,CD8+更低(P<0.05)。两组不良反应发生率未见统计学差异(P>0.05)。结论:右美托咪定联合罗哌卡因ESPB对肋骨骨折患者术后的镇痛效果良好,可改善患者认知功能、免疫功能和应激反应水平,治疗安全性较好。

      Objective: To investigate the changes in postoperative immune function, analgesic effect, and stress response in patients with rib fractures undergoing erector spinae muscle plane block (ESPB) with ropivacaine combine with dexmedetomidine. Method: A total of 104 patients with rib fractures who were admitted to our hospital from June 2022 to June 2023 were randomly divided into observation group and control group, with 52 patients in each group. Both groups of patients underwent operative treatment, the control group received ropivacaine ESPB analgesia and observation group received dexmedetomidine combined with ropivacaine ESPB analgesia. The postoperative analgesic effects, cognitive function scores, serum stress response indicators, immune function and adverse reactions were compared between two groups. Result: Compared with control group at 1h, 3h, 6h, 12h and 24h after operation, the observation group had lower Visual Analog Scale (VAS) scores in active and resting states, and higher scores in Mini Mental State Examination (MMSE) (P<0.05). Compared with control group 24h after operation, the observation group had higher levels of serum cortisol (Cor), norepinephrine (NE), epinephrine (E), CD4+, CD3+, CD4+/CD8+, and lower levels of CD8+(P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: The combination of dexmedetomidine and ropivacaine ESPB has a good analgesic effect on patients with rib fractures after surgery, improves their cognitive function, immune function, and stress response levels, and has good treatment safety.
