目的:本研究聚焦于痤疮这一常见皮肤疾病,借助前沿的代谢组学技术,并深度融合生物信息学分析手段,深入剖析痤疮患者在不同疾病进程阶段以及不同性别群体中血清脂质的独特特征与差异表现。通过精准筛查潜在的生物标志物,旨在为深入探究痤疮的复杂发病机制开辟全新视角,同时为临床实践中痤疮的精准分级预测提供极具价值的创新思路与理论依据。方法:2021年11月至2022年8月于安徽医科大学第一附属医院皮肤科采集41例轻、中、重度痤疮患者血清,及26例健康对照组血清,采用液相色谱-串联质谱法(liquid chromatograph mass spectrometer, LC-MS)进行血清脂质组学分析。采用偏最小二乘法判别分析(partial least squares discrimination analysis, PLS-DA)对差异表达的代谢物进行多变量统计分析;采用火山图及基因本体(Gene Ontology, GO)通路富集分析对代谢组数据进行深入解析,探究不同进程及性别患者中显著改变的信号通路,并筛选关键的候选生物标志物。结果:本研究揭示了不同进程痤疮患者血清代谢谱的差异。在进程分析中,通过火山图发现不同进展阶段的代谢物变化类型不同。GO富集分析显示,痤疮进程中嘌呤通路始终显著上调,但各进程的显著失调通路不同:轻度和中度组以嘌呤通路最显著,重度组则以甲硫氨酸代谢通路上调最明显。在性别分析中,男性痤疮患者以嘌呤通路失调最显著,女性患者则以精氨酸和脯氨酸通路最明显。此外,通过韦恩图确定了9个进程共有的代谢物、6个各进程特有的代谢物,以及男女痤疮患者分别特有的9个代谢物。 |

Objective: This study investigates acne, a common skin disorder, by applying advanced metabolomics and integrated bioinformatics analysis to comprehensively characterize serum lipid profiles across disease stages and between genders. By accurately identifying potential biomarkers, this study aims to shed new light on the complex pathogenesis of acne and provide a theoretical foundation and novel insights for precise clinical grading and prognosis. Methods: From November 2021 to August 2022, serum samples were collected from 41 acne patients with mild, moderate, and severe acne, as well as 26 healthy controls, at the Department of Dermatology, First Affiliated Hospital of Anhui Medical University. Serum lipidomics analysis was performed using liquid chromatography-mass spectrometry (LC-MS). Multivariate statistical analysis of differentially expressed metabolites was conducted using partial least squares discriminant analysis (PLS-DA). Metabolomics data were further analyzed using volcano plots and Gene Ontology (GO) pathway enrichment analysis to identify significantly altered signaling pathways across disease stages and genders, as well as key candidate biomarkers. Results: This study revealed distinct serum metabolite profiles among acne patients at different disease stages. Volcano plot analysis identified stage-specific metabolite alterations, while GO enrichment analysis showed consistent upregulation of the purine pathway across all stages. The most significantly dysregulated pathways varied: the purine pathway predominated in mild and moderate acne, whereas methionine metabolism was most upregulated in severe cases. In gender-based analysis, males exhibited prominent dysregulation of the purine pathway, whereas females showed significant alterations in arginine and proline metabolism. Additionally, Venn diagram analysis identified nine metabolites common across all stages, six stage-specific metabolites, and nine gender-specific metabolites in male and female patients, respectively. |
